Printable Restraining Orders?

I want to get a restraining order that you can print out. I dont even really care if its legal or not. There is this guy at school, and he is a really BAD word, ok? So I just want this so he will leave me alone, so he thinks its legal and stuff. Do you know any websites where I can get one?
